Risk Assessment Report

Machine Risk Assessment Services

DAK Machine Safety provides comprehensive machine risk assessments in accordance with ANSI B11.0, ISO 12100, and OSHA 1910 Subpart O, helping manufacturers identify hazards and implement appropriate risk reduction measures to safeguard personnel interacting with industrial equipment.

What Is a Machine Risk Assessment?

A risk assessment is a structured process that identifies:

  • Tasks performed on a machine
  • Associated hazards (mechanical, electrical, thermal, etc.)
  • Exposure frequency and severity – Risk
  • Existing safeguards and risk levels
  • Recommendations for risk reduction

This analysis helps ensure your machines are compliant with national and international safety standards while supporting productivity and ease of use.

Our Risk Assessment Process

DAK’s FS and Automation Engineers will perform an onsite evaluations by walking the plant machine, understand the tasks on the machine and see them interact with it as part of their day to day operation. We work closely with your operators, maintenance teams, and EH&S personnel to document how machines are used to ensure that our risk reduction measures bring the risk to an acceptable level and maintain efficient balanced safety and efficiency.

Our process includes:

  1. Machine Overview
Recording machine make, model, serial number, and operational location. Different personnel experience on various shifts, and seasonal time.
  2. Task & Hazard Analysis
Documenting tasks performed on the machine (e.g., loading, cleaning, servicing) and identifying associated hazards.
  3. Risk Estimation and Evaluation
Using industry-accepted risk rating methodologies (severity, frequency, possibility of avoidance) to assign a risk level. If the risk is acceptable or not.
  4. Risk Reduction Recommendations
Providing specific solutions such as design out measures, automation, interlocks, guards, control upgrades, signage, or alternative control measures or lockout tagout, procedures and trainings.
  5. Prioritized Action Plan
Highlighting high-risk machines and tasks that require immediate attention.

What You Receive

DAK delivers a professional Risk Assessment Report that includes:

  • Full documentation of each machine evaluated
  • Photographs of hazards and existing controls
  • Task-by-task hazard identification
  • Assigned risk scores before and after mitigation
  • Recommended engineering and administrative controls
  • A clear roadmap for achieving compliance and reducing risk
  • Safety requirement specification to identify the hazardous outputs / span of control for each input safety control device, the required PL for the safety function,
  • Visual Report of the Solution
  • Management Report
